Job Description:
HumanEdge Skilled Nursing and Allied Health is seeking a local
contract nurse RN Case Management for a local contract nursing job
in Omaha, Nebraska. Job Description & Requirements
- Provide case management services, including care coordination and
discharge planning, for patients in a rehab setting.
- Act as a liaison between patients, families, physicians,
interdisciplinary teams, and insurance providers.
- Ensure timely communication and documentation related to patient
care transitions.
- Help patients and families navigate the healthcare system and
access appropriate post-discharge services.
Requirements:
- Current licensure in a clinical discipline per Nebraska state
guidelines (RN, RT, or Social Worker preferred)
- Associate's Degree in a healthcare field (Bachelor's degree
preferred)
- Minimum of 2 years of experience in case management and discharge
planning in a rehabilitation care setting
- Strong working knowledge of the insurance industry and government
reimbursement programs
- Excellent communication, organization, and problem-solving
skills
