Hillcrest Health Services is currently seeking an RN Case Manager/Registered Nurse to join our Home Care team in Lincoln, NE.The RN Case Manager/Registered Nurse provides case management oversight and direct client care services in the client home or alternative setting for home by utilizing the nursing process and clinical knowledge to ensure high quality care.This is full-time position with benefits is eligible for a $3,000 retention bonus!Schedule: Daytime hours and flexible schedule!Our goal at Hillcrest is to inspire people to live their best lives.We are committed to providing a career that lets you thrive and grow.What does Hillcrest have to offer? Delivers high quality nursing care including proficiency with direct skills such as head-to-toe assessments, wound care, IV, catheter management, disease process education, medication management, etc. Performs necessary documentation in the EMR according to agency policy including consent for treatments, admissions, evaluations, assessments, plan of care development, prior level of function, discharge planning and ongoing physician orders. Collaborates effectively with the case manager by delivering the right visit/intervention at the right time. Demonstrates ability to critically think, plan, organize and problem solve around the ongoing needs of the client.What do I need to be considered? STAR mentality (Selfless, Tough, Accountable, Respectful). At least one year experience as a nurse in a home care setting within the last 5 years preferred. Must be licensed in the state of Nebraska or compact-state as an RN.
