collaborating with a multi-disciplinary team, including PT, OT, ST,

You will supervise your caseload, nurturing the growth and
development of LPNs and Home Health Aides, as we cultivate a team
of caregivers. In your role as a facilitator of care coordination,
we celebrate the unity that arises when PRN staff steps in,
ensuring seamless support for your patients.
Requirements include an Active RN License, one year of clinical
excellence, CPR Certification, and a valid Driver's License.
