Employees are held accountable for job performance. This position provides care to assigned residents in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized resident care plans.
Completes the assigned portions of the MDS for Nursing. Maintains residents self-esteem, privacy and confidentiality of personal information. Assists to maintain safe, pleasant, orderly resident living areas. Completes nursing portion of MDS and reviews for completion and validity of entire process. and provides final quality check prior to MDS transmission. Observes and reports residents conditions. Communicates with physicians using center/campus systems. Directs and assists with co-worker, resident, family/significant other education. This position will be held accountable for complying with all related laws, regulations, company policies and procedures pertaining to his or her position and for fulfilling his or her obligations under the Society s Corporate Compliance Program.
Graduate of an accredited school of nursing. RN nursing licensure for state of practice is required. Must be certified through AANAC as a Resident Assessment Coordinator (RAC-CT) within 6 month of hire.

Must be able to effectively communicate in English, both orally and in writing.

Graduate of an accredited school of nursing
- Must be certified through AANAC as a Resident Assessment Coordinator (RAC-CT) within 6 months
- 6 of the AP agreement, a $500 fee will be billed if your clinician backs out or quits with less than a 14-day notice.
